Community Paramedicine – Social Worker

Full time position (40 hours/week)

Job Description

Through federal funding, the Crawfordsville Fire Department has an opening for a social worker to be part of their Community Paramedicine program.  This position is being funded from this grant for the period of September 1, 2021, through August 31, 2024.

This grant funded program provides education, training, and coordination of services for those who suffer from substance use disorder (SUD), their caregivers, and the community.

Responsibilities include but not limited to:

  • Collaborating with multidisciplinary teams to evaluate and assess patients’ medical and physical needs.
  • Refer patients to appropriate services if patients are identified with SUD.
  • Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficacy.
  • Assist patients and their caregivers in identifying goals of care and assists with unmet needs through linkages to community resources.
  • Provide oversight to the group of practicum social worker students, who will be supporting the paramedicine program in providing on-site assessments and referral of services.

Qualifications

  • Licensed Social Worker or a Master’s degree in Social Work
  • One year management/supervision experience.
  • One year experience in social services within the SUD field.
  • Ability to maintain composure, even in difficult situations
  • Manage conflict and tactfully handle complaints and difficult situations.
  • Critically evaluate and analyze information for reporting purposes.
  • Develop effective relationships with patients, hospital departments, and other consortium members.
